SERVICES AT HOME

At Dynamic Rehabilitation, our experienced health professionals are able to assess and treat clients directly in their home. This allows us to make recommendations that are directly applicable to the client and his/her home environment.

Case Management

Our case managers meet with the client in his/her home to establish a rehabilitation plan, implement plan recommendations, and monitor the client’s progress.

Occupational Therapists

Our occupational therapists are skilled in evaluating our client’s abilities to complete his/her activities of daily living at home, including the areas of personal care, housekeeping, home maintenance, and care giving.

Our therapists also complete assessments for assistive device prescription, home modifications, and home safety and are able to implement their recommendations. For clients with acquired brain injury, our therapist’s complete functional cognitive assessments to determine how cognitive limitations impact the client’s participation in his/her activities of daily living.

Physical Therapists

Our physiotherapists assess and treat clients at home when clients cannot attend a clinic, require one-to-one therapy, or when it is more cost-effective or beneficial to the client to receive services at home.

After Assessment

Our team is skilled in developing rehabilitation and return to ADL programs that specifically address barriers from a physical, psychosocial, cognitive and/or environmental perspective. Programs are goal oriented, related to pre-injury/pre-illness activities, and tracked daily by the client. Day to day activities are used as therapy whenever possible to promote meaningful progress.
