SERVICES AT THE HOSPITAL

At Dynamic Rehabilitation, our experienced health professionals begin the rehabilitation process while our clients are still in hospital. By ensuring that our clients discharge home safely and with needed services, our clients are reassured that their needs are taken care of and that any unnecessary delays in their rehabilitation are minimized.

Case Management

Our case managers will visit the client at the hospital and communicate with the hospital team to understand the client’s current medical status and prognosis, and begin coordinating services to promote a safe and successful discharge from hospital.

Occupational Therapists

Our occupational therapists will visit the client at the hospital and communicate with the hospital team to understand the client’s current medical status and prognosis and determine his/her current level of function with activities of daily living. Often, they will visit the client’s home to complete an environmental assessment to identify any safety concerns.

Occupational Therapists assist with safe and successful discharge from hospital by making recommendations when appropriate for:.

• Activities of daily living that the client can complete independently.
• Activities of daily living that the client will require assistance with to complete.
• Assistive devices, equipment, or home modifications required for client’s safety, pain management and/or to promote independence with activities of daily living.
• Alternate discharge destinations if it is unsafe for client to return home.

Other Services & Rehabilitation Coordination

Our health professionals can also complete specific tasks, such as organizing a specific service or equipment, as needed.

Other professionals, such as physical therapists and kinesiologists may also assist in the transition from hospital to community and can provide specialized services to facilitate discharge as needed.
